All UC San Diego students are eligible to join the UC San Diego Badminton Team by showing up during team practices. All members must pay an annual membership fee and complete a waiver form.

If this is your first time, the log in is your UC San Diego email address and the password is the same as the one for your email account. Please fill out everything carefully because you will not be allowed to play if there is any mistakes in the form. Here are things to remember:

• Do not forget to initial and sign all the waivers.
• If you are under 18, request and sign a minor waiver form from one of the captains during practice.
• Do not forget to include your doctor's name and phone number.
• Your emergency contacts must be within the San Diego county. You may put your roommates as your emergency contacts.
